File not found (icon_######)

If you're reciving a file not found error in your Teamspeak 3 window it means you're custom icons are referencing an image that no longer exists on the servers.

To correct this:

1) Open TS3 client

2) Go to Tools, Edit Virtual Server, Click on "Icon"

3) You obtain a new window to delete or upload icon here to fix the missing ones.
